 We are currently seeking a dynamic team player to join our team as a:

CLIENT SERVICES SUPERVISOR
(Visiting Home Health Care - Hamilton Area)

The Client Services Supervisor is responsible for:

• monitor the delivery of client service by personal support workers to minimize risk and promote safety

• provide supervision to personal support workers including monitoring performance and disciplinary action

• facilitate adherence to organizational policy

• provide individual on site education as required

• liaise and maintain communication within Client Services team

• visiting clients in their home to assess safety of client home environment, verify client services as per client care plan

• attend case conferences as requested

• maintain appropriate verbal and written reporting to meeting reporting requirements

• document issues and report to appropriate source while maintaining confidential records

• monitor appropriate use of resources within areas of responsibility

Qualifications:

2-3 years of experience working in community nursing. Current certificate as a Registered Nurse from the College of Nurses. Possess a thorough understanding of professional standards of practice and code of ethics. Demonstrates commitment to ongoing improvement in client care. Bilingualism or ability to speak other languages is an asset. Valid driver’s license and vehicle required
